A brand awareness survey (also called a brand awareness questionnaire or brand tracker survey) measures how well your target market recognizes and remembers your brand. It's the foundational market research tool marketers use to understand whether consumers know you exist — a prerequisite for consideration and purchase. This brand awareness survey template measures all four awareness dimensions: (1) Unaided awareness — respondents name brands unprompted ('When you think of [category], which brands come to mind?'). (2) Aided awareness — respondents recognize the brand from a list. (3) Brand knowledge depth — how much they know about the brand. (4) Purchase consideration — likelihood to choose the brand. Aided awareness is usually much higher than unaided; the gap reveals marketing effectiveness.
Brand recall (unaided) asks respondents to recall brands without prompting — a strong signal of true top-of-mind awareness. Brand recognition (aided) presents a list and asks which brands they recognize — easier to answer and always yields higher numbers. The best brand awareness questionnaires include both: start with unaided recall to avoid biasing answers, then follow with aided recognition for a full picture.
